The federal government program, applicable to this Covid-19 portal via which you registered and may have been tested for Covid-19 or may have received a vaccine for Covid-19, ended on May 31, 2025; and access to any of your applicable records via this portal will end at 11:59 p.m. Central Time on December 19, 2025.  You may obtain a copy of your applicable records any time prior to 11:59 p.m. Central Time on December 19, 2025, by logging in to your account here, accessing your records, and downloading a copy to your own device, which you may then save or print.  If you have questions related to your records, send an email to Covid19@etruenorth.com before 11:59 p.m. Central Time on December 19, 2025 for more information.

After December 19, 2025, to obtain a copy of your records, you will need to visit www.mycoviddata.info to submit a request to the Custodian of Records for the site. 

Please note that this site primarily contains registration data.  Certain records, such as test results, are not necessarily retained on this site, and you may need to separately contact the healthcare provider (such as the laboratory that processed your test) or public health agency to obtain such records.
